Traditional and Ultrasonic Techniques
- Traditional Rhinoplasty involves reshaping the nasal bone and cartilage using surgical instruments like chisels, rasps, and hammers. The surgeon manually adjusts the bone and cartilage, which can sometimes lead to more trauma, bruising, and swelling. This method relies on the surgeon's experience and tactile feedback to achieve the desired outcome.
- Ultrasonic Rhinoplasty (also known as Piezo rhinoplasty) is a more recent technique that uses ultrasonic (high-frequency) vibrations to reshape the nasal bones. A specialized device, known as a piezotome, is used to precisely cut or shave the bone without affecting the surrounding soft tissues and cartilage.


Open and Closed Rhinoplasty
Both traditional and ultrasonic rhinoplasty can be performed using either an open or closed approach. The choice between open and closed techniques depends on the patient's specific needs and the surgeon's preference.
- Open Rhinoplasty involves making small incisions in the lower part of the columella (the lowest part of the nasal septum, located between the two nostrils). The skin and soft tissues are lifted away from the underlying nasal structure, allowing the surgeon to make the necessary adjustments. This type of Rhinoplasty is typically chosen when there are issues or limitations in performing a Closed Rhinoplasty. The advantages of Open Rhinoplasty include greater visibility for the surgeon, as well as the ability to make more complex changes and adjustments to the shape of the nose. However, this method leaves a small scar, and the recovery period is longer.
- Closed Rhinoplasty involves making the necessary corrections through incisions inside the nose, on the mucosa, which is the main advantage of this approach. The cartilage and bone are separated from the skin, and portions of bone and cartilage can be removed, reshaped, augmented (through grafting), or restructured to achieve the desired form. Performing the procedure through internal incisions ensures that no external scars will remain on the nose or face. Absorbable sutures are typically used. The Closed method is less traumatic to the nose compared to the Open method, leading to a quicker recovery.
